ECAPS WATCH

A knowledge hub for pharmacy industry professionals

Be reimbursement-ready on day 1 for ECAPS

What Is ECAPS?

The Ensuring Community Access to Pharmacist Services Act (ECAPS) is bipartisan legislation (S. 2426 and H.R. 3164) designed to recognize pharmacists as providers under Medicare Part B.

Once passed, it will enable pharmacists to receive direct reimbursement for test-to-treat clinical services (such as influenza, COVID-19, streptococcal pharyngitis, RSV).

By granting pharmacists provider status in Medicare, the bill opens a pathway to large-scale reimbursement, paving the way for broader payer adoption.

ECAPS Is a Serious Inflection Point for Large National Pharmacy Chains

Expanded Access for Patients

90% of Americans live within 5 miles of a pharmacy that they visit once every 30 days

Filling Care Gaps

Expanding the role of clinical pharmacists helps patients find timely access to care when primary-care provider availability is limited.

Competitive Advantage

Pharmacies prepared to deliver and bill for these services will stand out in a crowded retail healthcare market

New Revenue Streams

Direct reimbursement for test-to-treat clinical services such as influenza, COVID-19, streptococcal pharyngitis, and RSV.

Timing is crucial.

When ECAPS passes, pharmacies that are ready on day 1 to deliver and bill for services will capture the greatest advantage.

Waiting to build infrastructure after the law takes effect means lost revenue, slower patient adoption, and potentially losing patient volume to competitors.

 FAQs

  • The Ensuring Community Access to Pharmacist Services Act (ECAPS) is bipartisan legislation (S. 2426 and H.R. 3164) designed to recognize pharmacists as providers under Medicare Part B. Once passed, it will enable pharmacists to receive direct reimbursement for certain clinical services (such as influenza, COVID-19, streptococcal pharyngitis, RSV).

  • For large chains, ECAPS opens new revenue streams by reimbursing pharmacists for patient care services. Pharmacies that are ready to implement and scale these services will see increased patient access, stronger community ties, and improved margins.

  • When ECAPS passes, the pharmacies best positioned to bill and deliver services immediately will gain a first-mover advantage. Waiting until after passage to build infrastructure means lost revenue, delayed market entry, and possible patient leakage to competitors.

  • ECAPS is focused on Medicare beneficiaries.

  • ECAPS would cover test-to-treat services for flu, strep, Covid, and RSV for Medicare patients.

  • Workflow Services provides turnkey infrastructure for clinical services — including billing enablement, workflow design, technology integration, staff training, and scalable rollout support. We ensure your pharmacies are ready to capture reimbursement on Day 1.

  • No, ECAPS doesn’t replace or change dispensing. Instead, it adds new reimbursable care opportunities that can be layered into pharmacy operations without disrupting prescription fulfillment.

  • As soon as the bill passes and CMS issues implementation guidance, pharmacies prepared with billing and clinical workflows can begin submitting claims. Early movers are positioned to capture revenue fastest.

  • Initially, yes. ECAPS applies to Medicare Part B beneficiaries. But history shows that when Medicare establishes pharmacist reimbursement, commercial payers and Medicaid programs often follow suit.
