The Main Street Pharmacy Access Act

A knowledge hub for pharmacy industry professionals

Be reimbursement-ready on day 1

What Is the Main Street Pharmacy Access Act?

The Main Street Pharmacy Access Act (formerly named the Ensuring Community Access to Pharmacist Services Act or ECAPS) is bipartisan legislation (S. 2426 and H.R. 3164) designed to recognize pharmacists as providers under Medicare Part B.

Once passed, it will enable pharmacists to receive direct reimbursement for test-to-treat clinical services (such as influenza, COVID-19, streptococcal pharyngitis, RSV).

By granting pharmacists provider status in Medicare, the bill opens a pathway to large-scale reimbursement, paving the way for broader payer adoption.

The Main Street Pharmacy Access Act Is a Serious Inflection Point for Large National Pharmacy Chains

Expanded Access for Patients

90% of Americans live within 5 miles of a pharmacy that they visit once every 30 days

Filling Care Gaps

Expanding the role of clinical pharmacists helps patients find timely access to care when primary-care provider availability is limited.

Competitive Advantage

Pharmacies prepared to deliver and bill for these services will stand out in a crowded retail healthcare market

New Revenue Streams

Direct reimbursement for test-to-treat clinical services such as influenza, COVID-19, streptococcal pharyngitis, and RSV.

Timing is crucial.

When the Main Street Pharmacy Access Act passes, pharmacies that are ready on day 1 to deliver and bill for services will capture the greatest advantage.

Waiting to build infrastructure after the law takes effect means lost revenue, slower patient adoption, and potentially losing patient volume to competitors.
